On 11/22, speak with Yosuke in the 2-2 Classroom.
On 11/23, speak with Chie in the Shopping District, North.
On 11/24, speak with Rise on the 1st Floor of the Classroom Building.
This achievement is very missable, but these dates are not set in stone, if you happen to miss one of the dates don’t panic. You can always visit her later by speaking to the above characters but she’s only in the hospital until

You really have until November 25th and you only get one visit per day. Finals start on the 28th and lock you out of any more interactions from there on out.
You also cannot visit the hospital with a character if you have a pending social link event with them. I tried unlocking this with a save starting on the 25th and could only make the trip once with Chie (whom I had maxed) on the 27th.
So if you miss the 22nd deadline and don't have Yosuke's, Chie's, AND Rise's links maxed, you'll have to wait for your New Game+ run.
You also cannot visit the hospital with a character if you have a pending social link event with them. I tried unlocking this with a save starting on the 25th and could only make the trip once with Chie (whom I had maxed) on the 27th.

All of your Classmates will NOT have the ! Mark above their head when you can visit Nanako. But they will ask if you want to see Nanako today.

When you get deep into November, events will happen and Nanako will end up in hospital. As of November 22nd, you and one of your teammates can visit the hospital and check in on your lil’ sis.
On the 22nd, you’ll have to speak to Yosuke in your classroom to initiate the visit. On the 23rd, you can speak to Chie in the Shopping District (near Aiya's) and go with her to visit Nanako. And on the 24th, speak to Rise on the 1F of the main school building to go. Once you’ve done all three you’ll get this achievement.
It’s worth bearing in mind that these events will grow your relationships with the subsequent teammates, but if you’re short on time and going for Legend of Inaba (35G), you might not have time to spare. So, prepare a save beforehand and then reload the save when you’ve got the achievement.
