How to unlock the The Nose Doesn't Always Know achievement in Persona 4 Golden - Definitive Guide
Surprise Fusions occur at random. It could take you hours to get this to happen or it could happen in your first few fusions (thanks to a few comments on this guide for confirming this)
The best way to do it is by saving outside of the Velvet Room and just keep fusing and re-loading until it pops. The normal cinematic will begin and then suddenly the card will fall to the floor, unexpectedly. Igor will be shocked and you will receive a persona you did not expect.
Reload your save after you get the achievement.

do they only occur from choosing the personas yourself?
Fused around 200 times so far and had zero Surprises, I primarily use the first option which shows the outcome easily.

Honestly this is RNG based It took me well over 100 fusions for this to happen! What a drag it was
Apparently, fusions that take 3 personas are more likely to trigger this though.
What's funny is that on the vits, it happened a fair bit.

When you take two Personas (or more) in a Persona game and attempt to fuse them together, sometimes you'll experience either a lot of luck, or some terrible accident - depending on the outcome.
That random happenstance I’m referring to is called a "fusion accident." It basically means that the Personas you're trying to fuse won't give the expected outcome, and instead you'll get something else completely different. Fusion accidents are incredibly rare, but if you're going for Thorough Bookkeeper (70G) there's a good chance you'll get it (a two Persona fuse has a 1/64 chance of resulting in a fusion accident, while a 3 Persona fusion has a 1 in 32 chance).
If, somehow you don't trigger a fusion accident, then you just need to keep fusing. If, by the end of the game you don't have it, the best way is to create a save file, and then keep buying and fusing to your heart's content, and if you don't get it, just reload it and try again. If you do, great. You can still reload though, as it doesn't give you anything but a nice shiny achievement.
