How to unlock the The Deviated Cognition achievement in Persona 5 Royal - Definitive Guide

This achievement relates to a floor lacking in light in one of the floors in the Mementos Palace. You'll first encounter this change when you venture in the Chemdah portion of the dungeon, which is unlocked after you clear Madarame's Palace in the main campaign.

Definitely some randomness involved with this one. This appeared for me after entering Aiyatsbus: Area 2 before finishing Madarame's Palace. It'll be obvious when you reach your first deviation because a cutscene will play informing you about them.

My achievement for this popped right after that dialogue cutscene finished. You will also know right away if you'll get the achievement if the Floor of Mementos you are on is extremely dark, and there will be no mini map to keep track of where you've been on that floor.

It's also worth noting that (for me at least), when I got this achievement, that a Reaper ended up spawning on the same floor of Mementos I got this achievement on (without leaving the area of course). Defeating the Reaper is another achievement. Now, I was no where near prepared for this as I was only Level 20, and still prepping for the 2nd Palace - needless to say, I got owned, died, and had to start from the beginning of Mementos.

I'm not sure if the Reaper spawns every time there is a Deviation in Mementos as I've only experienced the Deviation once so far (when I got this achievement), but I figured I should at least warn others who may not be prepared to fight a Reaper. If you are underleveled when you get The Deviation Cognition achievement, get off the floor as soon as possible so you don't lose any progress. If you die, you'll have a choice to either load a previous save, or start from the beginning of Mementos.

I knew there was a "special" Shadow on the Floor I was on as there was an objective that said "find the Hidden shadow", I just didn't know the Hidden Shadow was going to be a Reaper.

A deviation is a random occurance in Mementos where a floor will be completely dark when you arrive, and you will not be able to see or use the mini-map. In trade, these floors have tougher enemies for better rewards, and a higher chance of treasure demons spawning. You will get the achievement the first time this occurs, so you can always just leave the floor and return to reset it to normal if you don't want to deal with the darkness.

*MISSABLE*

Mementos deviations are new random occurrences within Mementos. It is entirely random when they occur, but they do occur somewhat frequently, around five to ten times per playthrough. This trophy requires you to enter one for the first time.

When entering an area of Mementos, there is a small chance that the entire area may be blacked out and the mini-map will not fill in with the areas you've visited, instead showing nothing except for your icon. When encountering it for the first time, Morgana will inform you that both negative and positive effects are heightened. For example, Treasure Demons appear more frequently, but so do Strong Shadows.

Leaving the deviation area will cause it to disappear. If you do not want an area to be a deviation, then you can go back up to the floor you came from and then come back down again to reset the floor generation and make the deviation disappear.

The trophy will unlock as soon as you encounter the Mementos deviation for the first time.
