How to unlock the Accidents Happen achievement in Persona 5 Tactica - Definitive Guide

This achievement comes when you have a cutscene where your planned fusion for a new Persona in your roster goes awry in the Velvet Room as Lavenza tries to solve the problem, and it ends with you getting a completely different Persona instead. This will come at random as you perform fusions on a regular basis to both fill out the Compendium as well as bolster the strength of the overall roster for which to equip Personas on your party members.

After being first introduced to Persona fusions by Lavenza (where you'll earn the Lavenza's Aid achievement after performing your first one), you can press cn_start to skip a fusion animation sequence after you finish your initial fusion. You'll know this achievement is unlocked when you can't skip even after pressing cn_start in the lead-up animation. The achievement should pop on screen after the fusion accident animation is done playing. Again, it'll come up at random, but I'm just putting out an important visual cue to take where you know this achievement is unlocked as you get acclimated to fusing.

Unsure if it'd count but an easy way to get an accident to occur is to forge 2 weapons in 1 session. I noticed that doing so always resulted in an accident past the first forging. Unsure if it'd count for the chievo though as I already had it by the time I discovered this

I think this achievement is strictly for Personas, but it's possible I could be proven wrong. (I haven't got to the point of being taught weapon fusions yet...I'm still in Kingdom 1 [Lady Marie].)

Like the Collection of Knowledge trophy_gold.png trophy which is awarded for completing the Persona Compendium, it wouldn’t be a Persona trophy list without a “fusion accident” trophy - the worst of the worst, although this time, it’s not too bad.

A fusion accident in Persona 5 Tactica is a random chance occurrence that happens when you’re in the Velvet Room and attempting to fuse Personas together. What occurs in a fusion accident is that instead of getting the Persona you’re trying to make, you get a random one instead.

The ‘Accidents Happen’ trophy is pure luck, then, and if it’s anything like previous games, there’s a 1 in 64 chance of it happening with a 2 Persona fusion.

That said, in previous games you’d only be able to carry a handful of Personas and you wouldn’t unlock Personas as mission rewards, meaning you’d be able to do fewer Persona fusions - or at least, it'd cost you a small fortune and take you a lot longer to do the same amount. 

So, because in Persona 5 Tactica you get Personas as end of battle rewards, and you can hold a lot more (and more as the game goes on), this ultimately means you can spam Persona fusions until you get a fusion accident.

I actually got it while at 22% completion of the Persona Compendium, while fusing Personas to fill it out, and probably got it another 20 times (annoyingly) on my way to 100%.
