You can activate an All-Out Attack if you attack an enemy if he is not in cover or downed i.e. Grenadiers. When done you can attack again and you will get into One More state.
As soon as this state has been reached just place your party in a triangle formation around the corresponding enemy you have attacked before. If doing correct the triangle will turn into flames and it will be marked as Triple Threat. Just press
to activate All-Out Attack.
Do it 50 times and the achievement will be yours.

If you don't have this by the time you reach Mission 52, you can grind it out very quickly here.
Simply attack the open enemy with Joker (Or whatever the first character is) to trigger the One More and then move to a position to perform the All-Out Attack.
Once that is complete, press
and hit Retry, and repeat until it unlocks.

The “All-Out Attack” mechanic in P5T is kind of similar to that of its main game counterpart, but with a twist. To perform one you first need to down an enemy.
You can do that by first staggering an enemy to dislodge them from cover, so that they can no longer resist your attack - you can do this with most kinds of Persona skills. Enemies also stood out in the open, not behind cover, are also in this state.
When the enemy is standing quivering on the spot (we’re talking the standard enemies here, but you can also down other enemy types too), they’re now free to be taken down. You'll know when you can down an enemy because it’ll say "Get Em!!" above that character when you target them. Down them and this will give that character a “One More” - see You're Wide Open!

Once they're downed, you'll want to surround them with your active squad mates, so that the downed character is in the middle of the triangle that will appear between all 3 active squad mates - now that you’ve downed an enemy. You’ll know when you’ve done it correctly and can activate an All Out Attack when the triangle will set on fire.
When the triangle is on fire, you'll finally want to go back to the character (or any character, in fact) who has downed a character and then hit the
button. Once you confirm the attack, you'll perform an All-Out Attack, which does fantastic damage to anyone within that triangle.
