How to unlock the Never Forget achievement in Phantom Breaker: Omnia - Definitive Guide
By Completing Waka's Story and Bad Ending you will unlock the following 2 Achievements :
Then, to Battle Grounds (Watched Waka's bad ending)
Never Forget (Played the game on September 14th or cleared Waka's Story Mode)
Method :
First head to game options and change options to the following :
Round Count : 1
Time : 90
CPU Difficulty : Very Easy
Now go to Story and Select Waka.
When you get to Tokia Lose the match and then Continue.
Finish the rest of your opponents and after the final character you will unlock both Achievements.
Quick Summary :
Play Waka's story , lose once to Tokia and continue , finish the rest of the story and both Achievements will unlock.
Then, to Battle Grounds (Watched Waka's bad ending)

Just a small correction: there is no way to adjust CPU difficulty in Story Mode. You can only adjust it in VS and Single modes. So you have to be quite competent to complete Story Mode - it can be actually quite hard due to some characters spamming moves constantly.

This trophy will be unlocked upon fulfilling any one of the following conditions:
If you want to unlock this trophy without having to play through Waka's episode in Story Mode, you will need to play any match from start to finish on September 14th. You can easily do this by going to the system settings on your console and change the date to exactly September 14th; Afterwards, play a match from start to finish with any character (preferably one whose trophy you haven't unlocked yet) and the trophy will unlock alongside the character you played as.
Story Mode method:
If you choose to unlock the trophy by normal means, you can do so by playing through Waka's episode in Story Mode. This is one of the few story episodes in the game with more than one ending, and as such, you should also go for Then, to Battle Grounds while going for this one. Her Story Mode episode consists of her fighting against the following opponents:
