How to unlock the Step into the Breach achievement in Phantom Doctrine - Definitive Guide

In order to unlock this achievement, you must be playing a mission that requires at least four agents. More than likely, you will be able to unlock this during an assault on a Beholder Cell as I did.

These missions come about when you have decoded enough of the random "Conspiracy Files" in your Analysis tab. In order to piece these threads together automatically over time, have one or two agents working in the analysis room at all times. Eventually they should link the threads, leading to a Beholder Cell being uncovered.

During the actual mission, you can bring five agents in a tactical role, so do that. Once on the ground, if you do not want to start the operation with a breach, but want the achievement out of the way, make a save. Then, move the four (or more) agents to the entrance(s) of the SAME room. I lined up an agent each on 3 windows and one on a door. Then execute the breach command with one of them. You will know you are functioning correctly when, even with the single command, you see four agent paths leading into the same room. When they all storm in you will get the achievement, whether there are enemies in the room or not.

I've been unable to perform a breach outside of the tutorial. Every time I attempt it, the button is greyed out and I can't activate it. My agents have firing capacity and are near doors/windows. Does anybody have any advice?

Using the Breach interface is not the most intuitive so here is a quick guide:
First, queue up your agents next to doors outside the room. When at least 4 are in position and they all have at least one movement and one fire point, click the breach button. Now left-click anywhere inside the desired room. This enters the breach planning mode. Now right-click on an enemy to assign that enemy to the current agent. A teal glowing line will appear between that agent and their target. Click on another agent and repeat the target selection process for them. Continue to assign targets to each of the agents, as well as choose what weapon they use by clicking just above their character portraits. When each agent has a target, the bottom middle box should say “Breach Order Prepared”. Click “Confirm” to start the breach.

Keep in mind that breaching imparts a significant damage boost, so even poor weapons can often result in kills using breaches. The Breach tool will show you the projected maximum damage. If using silenced weapons, the breach will not break infiltration mode unless there are enemies or civilians inside the room that are not killed. I can verify that civilian kills will count towards the total of four, as this unlocked for me on a breach of a room with 2 baddies and 3 civilians.
