In order to accomplish this, you need to have progressed far enough into the game to be in the Hideout and its submenus. From here, tab (
) over to the workshop and use some of your starting funds to buy Hideout upgrades (
). Eventually, after completing enough of them, the option to create the "Forger" room will be available. (I was able to buy the room on the fourth day in game, thus it does not long it acquire and is a necessary asset). Buy it and pass time until it is complete.
Once the Forger is complete, go its eponymous tab and if you select an agent, you will see options to customize the agent's appearance
or create a new identity for them 
. I did the former and then the latter, but I believe the option is the only necessary one. Given that, create a new name, codename and nationality/residency for your agent and pay the requisite fee. Achievement unlocked.
**A side note, I would only do this when an agent's heat bar, (next to their name) is full and they have the exposed status effect. Doing this prevents them from being recognized and killed in the field. Alternatively, do it at any point and waste the money.
