Custom difficulty will only be available once you hit Level 50. If you're Level 50+ do the following:
1. Load up either Singleplayer or Multiplayer (make sure you are the host)
2. Select edit difficulty
3. Scroll to the right until you reach 'Custom 1'
4. Edit the difficulty to your liking
5. Click 'Apply' and the achievement should unlock

After Creating A Lobby, Click On The Difficulty At The Top Of The "Ready Up" Screen.
Go Over To "Custom" And Change Some Modifiers. Start The Game, Achievement Pops.
