During Act III, you will be given the main quest Court of the Penitents. Enter the building you're told to, commune with the four available gods, and accept and complete all of their quests to unlock the achievement. The four gods and their quests are as follows:
Galawain - The Old Queen and the New King
Rymrgand - Into the White Void
Berath - A Servant of Death
Hylea - The Nest Above the Clouds
Take note, Hylea's quest involves one of the two dragons needed for another achievement that is currently bugged and they must both be killed without turning off your Xbox or losing connection to XBL. I recommend you plan ahead and leave this quest until after you can handle the Adra Dragon.

Seems buggy; I completed all 4 quests and received the boon/blessing of all 4 gods, but it didn't pop.
