OK what you need to do is look at the top of the table by holding X.
You need to hit the ball with the upper right flipper into the red lane at the top. If done correctly the ball will go down the big squiggly ramp on the left side of the table. 3 Ninjas will appear on the table. Launch the ball from the left side where the ball landed and when the ball is just about over the ninjas hit release which I believe is A on the xbox one. It's different than the 360 version which is why I failed this several times.
The easiest way I found to get the ball to the top right flipper is from a ball launch at the beginning of the game, but sometimes the ball doesn't make it to that flipper annoyingly. Hitting the ball to the right of the right blue ramp can get it there too or the left one. I accomplished it twice in the video.

This is a relatively easy achievement compared to most of the others on most of the other tables. You have to hit the ball with the Skill Shot flipper (top right) into a specific ramp. This will send the ball through the "adamantium chamber" AKA "mutagen chamber", which is the big greenish tube along the left of the table. The ball will exit the left side of the chamber and sit on a new "plunger". The "Hand Ninjas" will pop up in the middle of the table and raise their "hand shields" above their heads. Hold, then release, the 
button to fire the ball back up through the chamber and out the top. When your ball exits the top of the chamber it will slide from left to right across a set of rails. The goal is to release the ball from the rails and have it land on one of the "Hand Ninjas". When you are just left of any ninja press 
or 
to drop the ball. If you time it right, the ball will drop onto a ninja, the medal will spin in and the achievement will pop. I've included a video showing how I went about it.
