How to unlock the Builder achievement in Plague Road - Definitive Guide
There are only five different buildings that you need to unlock in your Farm. All five of them are unlocked via the same method - recruiting the related NPC while you're out adventuring.
Keep - Unlocked when recruiting Edwin. You will need an Engineer and a Soldier in your party to recruit him.
Machine Room - Unlocked when recruiting Bevis. You will need an Engineer and a Peasant in your party to recruit him.
Sanctuary - Unlocked when recruiting Lenore. You will need two Nurses in your party to recruit her.
Train - Unlocked when recruiting Bevis. You will need an Engineer and a Peasant in your party to recruit him.
Witch Tower - Unlocked when recruiting Astrid. You will need a Witch and a Soldier in your party to recruit her.
As you can see above, recruiting Bevis unlocks two buildings - so there are only 4 total NPCs that you need to recruit.
The order that they appear is different in each playthrough, but they always appear at predetermined spots.
The first and last screen of the Plains and the first and last screen of the Forest will house these characters.

In order to unlock all the buildings, you have to recruit special survivors who will not join up with the Doctor unless he has specific characters in his party. The location of these survivors changes through playthroughs but there will always be one at the beginning and end of the plains and forest.
Note: I had to back track on my second play through to find my last one, so if you can’t find one of these survivors, try fast traveling to the beginning of the forest map and back track to the last room of the plains map or fast travel to the outskirts. You do not need to develop any of these buildings to unlock this trophy.

Lenore: 2 Nurses – Unlocks the Sanctuary (Nurse training.)
Astrid: A Witch and a Soldier – Unlocks the Witch Tower (Witch training.)
Sir Edwin: Engineer and a Soldier – Unlocks the Keep (Soldier training.)
