This is in chapter 6 titled "The Fall" Near the ending!
***Possible Spoilers***
This Video (thx to MrBlasterMasterr) explains exactly what you do and it unlocks when you walk into the room with the life preserver.

I got lost when trying to find the elevator button, so I thought this was the way I was supposed to go, particularly because of the extra Cave Johnson quotes when you press the buzzers next to those large sealed doors. When I realised what the ship was (the one which popped up at the end of Half-Life 2 Episode 2) the awesomeness of this easter egg was confirmed, the achievement was just a nice bonus!

Chapter 6: The Fall

You will come to an area where you find GLaDOS as a potato and save her from a bird in the control room which unlocks the Tater Tore Achievement. You can get Ship Overboard before or after doing this. Above the Control room is another room. Shoot your portal on the wall near the catwalk that leads to the room. Go up there and enter the room. There is a secret door in the corner of the room. When you enter the room it is in the corner straight ahead to the left. Enter the door and continue down the hallway and exit the only door open. Walk up to the metal gate and the achievement will unlock.

You can also get it in Challenge Mode. In the map called "The Fall: PotatOS".

Just follow the other solutions from there.

Make your way through Chapter 6 - The Fall

You will come to a point in the chapter where there is 3 seperate levels to a control room. You must use your portals to gain access to the highest of the 3 rooms.

Once you're in that room. There is a half hidden door that you can pass through behind some cabinets. Go through that door and down the following hallway and out the open door at the end.

Move along the walkway to the left and the achievement will pop.

Here is a video to help with finding the hidden doorway.

Near the end of chapter 6 you'll come out in a large open area with a big yellow aparture sign hanging from the ceiling and an office located behind it in the top right of your screen. Portal up to the walkway leading to the office, walk straight to the far wall of the office and turn right 90 degrees to find a hidden door in the corner go through this door into a corridor with 3 of the vitrified doors required for this-

walk to the end of this corridor and walk through the open door on your right. Turn left and walk up to the life ring you can see on the floor. Trophy unlocked

to get this achievement, you will need to have got PotatOS in the fall or go to the PotatOS level in challenge mode. or follow the video!

Found in Chapter 6, after unlocking Door Prize. In the same office building where you found the last three doors, there's a doorway leading outside to a life preserve and the missing experiment. At 2:33 in the video, is where the experiment is located.

Chapter 6 &lt;FALL>

You will come to an area where there is a huge ball hanging high, and an elevator nearby. At the very top above the CONTROL ROOM office there is another small office. In this office there is a door behind the closet in the corner; go to it and it will open. Go inside the rescue circle and the achievement will unlock.
