Straight north of the "Potion House" (your home in the game) is a place called "Willow Waters Bathhouse". This place is opened from 09:00 to 18:00, every day except Thursday.
Inside there is an old till (cash register) that you interact with and when you agree to the prompt, you take a bath. It costs nothing, so don't worry about needing money.
It takes 2 hours to do. You can go right back in if you want to, but you can only do 4 a day.
Once you bathe a 10th time, the achievement will pop.
