You will unlock perk slots for your caddie as you level (lvl 35 for the 3rd perk). You must also get 3 perks from packs that you buy with the in-game money and equip them all to the same caddyie.
Another solution is to use a friend's caddie that already has 3 perks.

Which packs give the perks? I have been buying all 15k packs and haven't got any perks yet (out of or 5 packs purchased so far).

I got my perks in the 15k packs and the 60k packs. I believe you should be able to get them in any pack other than the booster. There are varying levels for each perk though.

When I got it I used a friend's caddie with 3 perks (drop chance, skip chance, slower meter) and after I finished a round with the caddie it unlocked.

Those are the exact perks my friend's caddie has equipped. Don't know what the issue would be.

Did not unlock for me with a friends caddie but as soon as I unlocked the third slot and equipped a perk it did. The friend method may have been fixed with an update.

Did you play a full 18-hole round? I did not try that. I did Par 5s & Par 3s rounds, but those are only 4 holes.

I did a 3-hole tourney & a 9-hole tourney after equipping a friend's caddie w/3 perks and the achievement never popped. So I decided to try Free Play instead, but this time before I started I pressed "Y" to select a caddie (even though I already had him selected). It popped as soon as I got to the section screen. I didn't even have to start the round. I hope that helps those having issues.

I just equipped my friends caddie with 3 perks at the golfer/caddie setup on the main screen and it unlocked. Didn't have to actually use the caddie.
