In general, you want to avoid shooting any enemies so that there's a lot of cars around you. This should mean that when the powerup does come along, that you can get as many kills as possible. Here are the settings I used which worked on my first attempt (after many failed attempts doing other stuff). I got double damage powerup, but a few of them would be good for this.
Level: 1-6 Extreme Relaxation
Weapon: FlameThrower Level 3 (aka Dr. Flim Flam's Hot Special 3000)
Buggy: The Second one (aka Hasty Buggy)
This is all you can probably afford and all I used. Basically run up the level without shooting things. You will take damage and might die. Just try to avoid as much damage as possible... this keeps as many cars around you as possible. Then about half way into the level, the powerup will appear. Try too shoot only it first, then hope for something like Double Damage.
I tried some other maps with magnet and boost combo, but could never get 18. This is largely luck based, so hopefully you didn't have as much trouble as I did. If you are having problems dying, you could probably use lev 2 flamethrower, and upgrade your car or other things for similar effect.
