You need to kill an enemy while using the Combat Focus ability. You can purchase this ability for 2 Neuromods. Then, select it from your favorites wheel by holding 
. It will cost 20 Psi points to use. Hold 
when you begin to fight an enemy (I'd suggest a mimic), and as long as you kill the enemy while still in combat focus, your achievement should pop!

It's worth noting that it doesn't have to be a designated enemy. Anything you can kill can be used to unlock this; I used Mikhalia.

use a neuromod to unlock Combat Focus, should only need the first level if you are on easy or normal and its located under Security, Then find a enemy, and pull the left trigger to activate Combat Focus and kill it
