Primal Survivors

This is cumulative and is extremely unlikely to happen naturally since this game is pretty easy. Simply grind it out by starting up a 15-minute game (so enemies are in greater number to make it quicker) and just get hit three times to die. Rinse and repeat until this unlocks.

Alternatively, if you have the Bones, buy the Dark upgrade that lowers your HP by 2 for improved damage. Now you'll die in one hit, making it very fast to grind this out.
