How to unlock the I Read A Book Once... Green It Was achievement in Prison Architect: Edition - Definitive Guide
You will first need to unlock "Education" from the Bureaucracy menu to be allowed to run education programs.
You will need a class-room with at least one Desk, although to maximize efficiency I would recommend 10 desks as that is the most you can use within one allotted session.(20 can be used for Foundation but only 10 for General) You will also need 1 office desk to meet the requirements for a classroom.
For prisoners to attend the General education program they will need to have completed the Foundation education program. They will need to attend 10 sessions of 3 hours each for Foundation course, and then they will need to attend 20 sessions of 3 hours each for the General education. There is a chance that Prisoners will not pass, but do your best to attend to their needs as the better they are treated the more engaged they will be with your reform programs.
The higher the punishment/suppression of a prison, the less likely prisoners will be to attend your reform programs as well. If prisoners are treated badly, surrounded by lots of armed guards, punished severely for slight mishaps in behaviour, then they will not want to attend classes.
Remember to set up the programs via the "Reports > Programs" menu and allow for at least 3 hours consecutively for work time to allow prisoners to attend. If you only have 1 classroom then you can only run either foundation or General at once, so the more class-rooms you have the more sessions you can run concurrently.
You don't need to have all 10 prisoners pass from a single session, you could have 2 pass, then 3 pass then 5 pass and you will get the achievement. The only other thing to mention is that you must do this on a Standard difficulty, so either County, Federal or Super-Max. This cannot be done on a custom difficulty.
EDIT: Mr Apple007 informs me that this achievement must be done within one Prison. You can still have them pass through multiple classes, but they must all be from the Same Prison.

There is currently a problem/bug 21/06/2021 where reloading a save file can cause all programs to disapear. So if you had say 4 days done with 4 sessions completed with 6 left to go. You save the game. Exit and save again.( has better survival than)" exit without saving"
You load the prison back up and your programs list is empty. ( this is the bug)
The 4 completed sessions are lost into ether. So you've then got to restart the whole 10 session foundation program again. I've yet to get anyone upto suitability for the general education course. But I suspect that could be game breaking.
If you don't care about deaths and or escapes your best bet is prolly just to hammer through 10 days whilst doing enough to not have the whole prison collapse into anarchy. That will get you your first X out of 20 past foundation. You can then attempt the general qualification but if you lose that program part way through I do not know if your eligible candidates are still available to try again.
I've experimented with saving manually, then saving on exit. Saving manually and not saving on exit, auto save increments adjusted. Auto save off and on but have not found a way to stop this problem from occurring. Some of the steam forums mentioned it occurred/ appeared with the going green update.
At the moment best advise is to try and do this in one session in its entirety without any reloading of save files.

I am sorry but I really do not know, I had all of mine in a single prison. I want to say you do need them done in one Prison but I can't verify that since I already have the achievement.

To piggy back off of FFX Brotherhood's point, I found it easier to use Leanne Johnston as the Warden on my buddy's GT. She increases the pass rate across all educational reform programs. Time to go get it myself!
