For those like me who struggled to get it on Prison architect mode (prisoners rioting too fast and/or not enough possible intakes), i found a really easy and fast way to get it.
Go in Warden mode and choose the Ironfort prison. The achievement is not difficulty related so you can take the easiest difficulty.
1) As soon as the game starts, fill the entire regime with yard time (Reports -> Regime), all the prisoners should go in the two yards.
2) Wait for about a minute or two, they will riot very fast.
3) Once you see that more than 50 prisoners are rioting, just keep hiring guards to stop the riot.
You will eventually see a red zone in a room, if that happens, just call Riot guards from the Emergency menu and send them in the zone to take it back.
The achievement should pop when the riot is over

This one is quite simple. I got it after doing some other achievements... 500 capacity and such.
Fill your prison with as many Max's as possible, I had a total of 500 inmates with about 120 being max. You could get away with far less but its what I had to work with.
1) You need a yard that is medium sized. Mine was about 16x30
2) Fence it off completely leaving a single block out so prisoners can get in.
3) Assign all your guards away from the yard, I put all mine in the canteen, this is to make sure that the guards don't end the riot before it reaches 50 inmates.
4) Fill your entire regime with yard time.
5) Wait until most of your inmates are in the yard and seal them in.
6) It won't take long before they kick off, and with a lot of Max's it will reach 50+ in no time
7) Send your guards in either through deployment or by re-assigning them to the yard.
8) Dismantle a portion of the fence to let the guards in and the inmates out
9) You can hire more guards if you have the cash and you think you need it. It wont take long for the guards to end the riot and the achievement will pop
It would help if your guards have stab vests and tazers.
