During your crossroads adventure, after you defeat the initial three enemies, two ghostly blue people will start wandering around town. Talk to one of them to start a side quest. Don't pick Squirrel farm. You'll go to a location, solve a puzzle or kill some enemies and return to the questgiver.
It's best to do this early, before you make the path to the enemy base. Otherwise, the world might be too full.

This did not work with the colored mystic stones quest as well. I returned the urn and nothing happened.

Try it again, maybe with a different quest. The programming on some of these achievements is buggy.
