How to unlock the Ouch IV achievement in Project Starship - Definitive Guide
This unlocks after you lose your 32nd shield. It is cumulative across all your games regardless of character or difficulty. You start with five shields. They are shown as icons across the bottom of the screen. Each time you pick up an S, you will add a shield until the horizontal bar on the bottom of the screen fills up with shield icons.
You lose a shield when you get hit. A shield icon will disappear from the horizontal bar on the bottom of the screen. Also, the playable character will say, “Ouch!” as an audio cue.
If you are hit with no shields in stock (or run into a wall with or without shields in stock), it will be game over. However, the system save file retains a count of the number of shields you have lost & resumes tracking once you start a new game. This achievement pops when the game's system save counter hits 32. This is invisible to the player because the Xbox's achievement tracker does not work with the game.

This is the 4th achievement in the Ouch series that one earns for losing shields. Accordingly, to get to the point to unlock this achievement, players should be quite familiar with the game. To reduce its challenge:
—Select Gwen for auto fire
—Choose easy difficulty
—Press continuously to slow down the action
Bear in mind, that you don’t want to make gameplay so easy that you never get hit—just easy enough that you are able to pick up a steady supply of shields between hits. Also, avoid the H for hardcore pickup. While it is active, you are prevented from using shields and, hence, cannot lose them.
If you have just started the game, and have played continuously, the Ouch achievements will unlock in order I through VIII. However, if you lost over 50 shields before Title Update for Project Starship came out &/or 66 shields before Title Update 2 for Project Starship came out, then this & subsequent achievements in the Ouch series will pop simultaneously and the date-time stamps may not be in order.
