The belived theory for this one is to beat level 11 on any difficulty. I did the level on easy and got the achievement (I honestly can't remember if I got any subobjectives or what my rank for the mission was).

he's right, just beat mission 11 on easy, doesn't matter if you got any of the sub-objectives or not - must be done in campaign mode though.
