This is done on level 6 and can be done on any difficulty (I did it on easy).
In this one, during the third phase of the mission Raymond will mention that he's in trouble but you should leave him and continue your mission. Ignore that, find him, and destroy every single last enemy around him until he thanks you for saving him. After that, just finish the mission as normal, and you should get the achievement.

FYI, you'll know you're on the right track if he says something like "I told you to leave me". Ellen will ask you what to do before that.

Good solution. Only thing I can add is that this is one of the subobjectives for the third phase of the mission, so you'll know you succeeded if it says "Subobjective complete: You saved Raymond!" after you're done clearing out his enemies.

Now this one confused a lot of people. No matter what happens Raymond gives you the charm after the mission however to get the achievement you have to save Raymond when he gets in trouble during mission 6 and tells you to not rescue him. It can be done on any difficulty.
