This one is gotten on level 16 (the final mission of the campaign). Just beat the level on any difficulty (I did it on easy).
When I did it, I completed all objectives, subobjectives, and got 100% kills (obviously since you're the only person in this mission), and did it in 4:12 and still only got a C ranking.
