UPDATE 02/11/2021: Thanks to the wonderful developers at Double Fine an update has been released which added a new Psychoseismometer in Otto's lab after finishing Gisu's quest and this includes Panic attack so this achievement is no longer missable!
Original solution:
In the 'PSI King's Sensorium' mission, which you gain access to by
(It was the 4th Brain level for me) You are introduced to an enemy type known as a Panic Attack which will first appear as a boss which will disappear after a set amount of time (Scripted in the story).
The "Phantom attack" this achievement refers to is when it splits into 5 with only one of the copy's being real and you must use your PSI Blast to hit the correct copy to earn the achievement (Thanks to Mecandes if you lock on using PSI blast the real Panic attack should be targeted). Later in the level after your first encounter with it you should receive the 'Time Bubble' ability which slows objects and enemies down - which allows you to defeat the boss when it appears again.
The reason this achievement is missable is because if you kill all the Panic attacks that appear in the boss arenas for this level - whilst the enemy type appears in later levelsthe achievement only counts the Mini Boss versions in 'PSI King's Sensorium'.
EDIT: People have reported unlocking the achievement in other levels but at some point in the story panic attacks don't appear again unlike the judges do for the Objection! achievement.
This means if you missed this achievement on the first run through of the game you will need to start a new save file, play up to the 'PSI King's Sensorium' mission and try again.
Personally, I got it first try in the limited time section when you are first introduced to the boss, its just best to wait for it to split into 5 and just shoot the copy you think it is. As long as you don't kill the all the instances of panic attacks in the game you should still be able to get the achievement on your save file.
NOTE: It is also beneficial to turn off "Narrative Combat" if you have the cheat turned on so you have more chances to hit it.
EDIT: The following has been debunked:
UPDATE: This is unconfirmed however the real Panic attack I hit was the one on the far left when Panic attack splits for the first time the first time it appears. Thanks to SquiSquiSquidio for mentioning that it was the same for them and that videos on YouTube also show the real boss is hit in the same place as well. Therefore aim for the far left panic attack when it first appears. (feedback on if this gives you the achievement so it can be confirmed would be appreciated)

When it splits into 5 do you have to hit the correct one first time or can you blast all 5 copies? Not at this point yet so don't know how the fight plays out.

After the casino mission you will re-enter the motherlobe. play through the missions inside the motherlobe until you start a mission to find a brain for the mail man to gain access to the mail room office. Your family will arrive in the reception and a cutscene will open up the quarry to you meaning you're free to explore outside of the motherlobe. once the cutscene is over you can see otto's lab directly opposite the motherlobe main entrance, go in and head to the machine at the back, and get a brain.
You will first face a Panic Attack in Psi King's Sensorium. It has a number of different attacks and moves very quickly, but you need to wait for it to split into 5 versions of itself. During this attack, you need to be quick and attack the real Panic Attack, you can attack the copies causing them to disappear as a process of elimination. The real version doesn't look or do anything different from the copies, so this does require some luck. Use Time Bubble and your Psi Blast abilities to attack the 5 versions quickly; hopefully, hitting the real one. You will need to restart the story if you have progressed to the end of the game as even going back to various levels won't allow the Panic Attacks to respawn.

The problem with this trophy is that, once you start encountering two at a time, they don't often use this attack anymore, if at all, and there is a finite amount of encounters with Panic Attacks in general. If you've cleaned every side-quest up in post-story, you won't encounter any more.
