UPDATE 02/11/2021: Thanks to the wonderful developers at Double Fine an update has been released which added a new Psychoseismometer in Otto's lab after finishing Gisu's quest and this includes Panic attack so this achievement is no longer missable!

Original solution:
In the 'PSI King's Sensorium' mission, which you gain access to by
(It was the 4th Brain level for me) You are introduced to an enemy type known as a Panic Attack which will first appear as a boss which will disappear after a set amount of time (Scripted in the story).

The "Phantom attack" this achievement refers to is when it splits into 5 with only one of the copy's being real and you must use your PSI Blast to hit the correct copy to earn the achievement (Thanks to Mecandes if you lock on using PSI blast the real Panic attack should be targeted). Later in the level after your first encounter with it you should receive the 'Time Bubble' ability which slows objects and enemies down - which allows you to defeat the boss when it appears again.

The reason this achievement is missable is because if you kill all the Panic attacks that appear in the boss arenas for this level - whilst the enemy type appears in later levels the achievement only counts the Mini Boss versions in 'PSI King's Sensorium'.

EDIT: People have reported unlocking the achievement in other levels but at some point in the story panic attacks don't appear again unlike the judges do for the Objection! achievement.

This means if you missed this achievement on the first run through of the game you will need to start a new save file, play up to the 'PSI King's Sensorium' mission and try again.

Personally, I got it first try in the limited time section when you are first introduced to the boss, its just best to wait for it to split into 5 and just shoot the copy you think it is. As long as you don't kill the all the instances of panic attacks in the game you should still be able to get the achievement on your save file.

NOTE: It is also beneficial to turn off "Narrative Combat" if you have the cheat turned on so you have more chances to hit it.

EDIT: The following has been debunked:
UPDATE: This is unconfirmed however the real Panic attack I hit was the one on the far left when Panic attack splits for the first time the first time it appears. Thanks to SquiSquiSquidio for mentioning that it was the same for them and that videos on YouTube also show the real boss is hit in the same place as well. Therefore aim for the far left panic attack when it first appears. (feedback on if this gives you the achievement so it can be confirmed would be appreciated)

When it splits into 5 do you have to hit the correct one first time or can you blast all 5 copies? Not at this point yet so don't know how the fight plays out.

Unfortunatly, I don't know as I literally got this on my very first shot so I couldn't test it but from the description I would assume it only counts if the first clone you hit after a spit is the correct one. If not this achievement would be rather trivial as I hit all five plenty of times in the fights.

I got it on the second one also. Use Psi Blast to get him to divide.

It does NOT lock onto the right one, but what you can do is lock and start shooting, the fake ones will disappear and you'll auto-target the next one; keep shooting until you hit the correct one and the achievement will pop. I did this on the second encounter with the boss in that level

It locked onto the correct one for me...

Edit: Every time I locked on to a Panic Attack, it locked onto the correct one. This is every fight.

You can get this achievement on the Psychoseismometer Quest aswell. For me I got several panic monsters to duplicate and the achievement unlocked when I hit the right one. For me it unlocked on the third device

i found that if you slow time down before they split the correct one will move at a slower speed than the copies also

There is a panic attack that will appear when you first enter Lucrecia’s mind and then again when you enter a second time

Easiest way is to just click the right stick to target, and it locks onto the real one, and fire like the guide says. Thanks!

If you missed this achievement on your first playthrough as I did, it took me just about 1hr to get back to the PSI King's Sensorium. You can turn on all of the assists and God Mode to speed on by. FYI, once you get to choose which Ford mission you want to go to, head over to the Mail Room first.

I can confirm you can still get this during the 2nd encounter with this boss. Just wait around long enough until the duplicates appear.

Is there really no possibility to fight against this enemy after doing all the missions plus side quests? I thought you can re enter all the main missions

I got mine as soon as I entered the first fight with it. Just waited for it to split and shot the left most version (the Youtube video I watched, said it seemed to always be this one, not sure if that's confirmed though)

Here's a trick and probably best to do anyway given the nature of Panic Attack. Time Bubble only seems to effect the REAL Panic Attack, so when he splits, just use Time Bubble in the direction of the speedy little demon and the one that turns black and white is your guy. Just shoot him and you're golden. It's how I got him on my first go.

The impression I got was that clicking right-stick to lock-on caused the real one to be selected. During my very first encounter with Panic Attack, it split a few times, and lock-on would select one; eventually I just fired at the locked-on one and the Achievement popped.

Thanks Mecandes! When i think back to when i did it i aimed for the centre one and it locked onto the far left - didn't even realize until you said it! I'll update the solution!

Tried to hit the far left one a few times with no luck. First time I hit the middle one it unlocked. So unless there was a delay in the achievement unlocking, it might be random which enemy to hit.

Did this on the first encounter with the boss. Just quit to title screen as soon as the cutscene starts where Raz says he don't know how to beat it because it's to fast. When you reload you're at the start of the battle.

Got it but was not the one on the left in the first battle. Second to right for me, second battle worked.

If you are worried about missing this or messing up you can enter the first fight at the start of the leveltry your luck and quit the game and resume to be put right back in hold B to skip the intro. I got it on the third try, it was the one on the far right fwiw.

I used targeting as well. If I missed I quit and restarted, I suck at combat so took me 5 tries. God mode has no affect on getting the achievement.

Glad to move on with the game and enjoy the rest without worrying about missed achievements anymore.

After the casino mission you will re-enter the motherlobe. play through the missions inside the motherlobe until you start a mission to find a brain for the mail man to gain access to the mail room office. Your family will arrive in the reception and a cutscene will open up the quarry to you meaning you're free to explore outside of the motherlobe. once the cutscene is over you can see otto's lab directly opposite the motherlobe main entrance, go in and head to the machine at the back, and get a brain.

You will first face a Panic Attack in Psi King's Sensorium. It has a number of different attacks and moves very quickly, but you need to wait for it to split into 5 versions of itself. During this attack, you need to be quick and attack the real Panic Attack, you can attack the copies causing them to disappear as a process of elimination. The real version doesn't look or do anything different from the copies, so this does require some luck. Use Time Bubble and your Psi Blast abilities to attack the 5 versions quickly; hopefully, hitting the real one. You will need to restart the story if you have progressed to the end of the game as even going back to various levels won't allow the Panic Attacks to respawn.

!! This trophy is missable. You have been warned. !!

Panic Attacks are introduced within the Brain in a Jar / PSI King's Sensorium level. They get more aggressive in the later levels, often bringing in two and some other enemies to attack you, so get this whenever you can. Your greatest chance is during the second sighting, after you've been given the Time Bubble PSI Power by PSI King himself.

When it projects four others with it and they're readying a projectile attack upon you, try to use Time Bubble to slow them down and then PSI Blast them. If you find the correct one, the others will disappear. The problem with this trophy is that, once you start encountering two at a time, they don't often use this attack anymore, if at all, and there is a finite amount of encounters with Panic Attacks in general. If you've cleaned every side-quest up in post-story, you won't encounter any more.

You could try and see if the Psychoseismometer side-quest given to you by Gisu may merit the Panic Attacks to perform the Phantom attack as well.

Attention! It is possible to miss the trophy! If you did not complete the passage on time, and also, in the collective unconscious, after the completion of the plot, during the journey through the brains of the characters, you could not do what was required - Panic Attacks will not respawn anywhere, you will have to start a new game.

Wrong. In Otto’s laboratory, where the chest was (you need to ride along the rails from the brain storage facility), there is a machine similar to the ones that were in the additional task. She throws on a site with a bunch of mobs, where, incl. The necessary bugs will actively spawn. You can repeat the battle ad infinitum.
