This achievement is unmissable.
When you start the game, you'll find yourself in what seems like a tedious office space, with Dr. Loboto being given Employee of the Year and a paid vacation. However, what you need to do is follow him, whereupon you'll go through several areas, with the help of the Psychonauts.
Once you complete this section, and find yourself in the real Psychonauts headquarters, the achievement will pop.

Managed to miss this one. Got every achievement in the game except this one. Figure it's just a glitch and it's easy to restart the game real quick and nab it. Just thought it was amusing that you said it was unmissable. Almost tempted to leave it locked just 'cause.

Once you pursue Loboto long enough, you'll come to a familiar setting with the Thorney Towers Asylum. All connection with the other Psychonauts that invaded will be cut off, leaving you to pursue him across the dangerous mindscape on your own. Once you ascend the ladder, you'll receive a cutscene, and after you leave the mind of Caligosto Loboto, you will earn this trophy.
