Apparently, I ran into a bug with this achievement. I've completed the game, and I have all achievements except for this one.

Achievements are bugged, I only missed the Duck, Duck, Goose one, but 9 others did not pop even though I met the criteria.

Story related and can’t be missed*
In Green Needle Gulch, play through Lucrecia's mind to help her unlock the memories which have been locked away by Ford Cruller. Assist Ford to break down the mental block to reveal the truth and return to the Heptadome.
Once the cut scene plays out the achievement should unlock.
* I had to play though twice to get this to unlock. Like others I have had several achievements glitch and not unlock. It seems that using Quick Resume on the series X is causing problems with unlocking achievements. Make sure that you quit the game completely each time you finish playing and don't leave this title in Quick Resume. All four of the achievements that didn't unlock for me on the first play through unlocked when not using QR.

Got the same problem i think, when you mean play though twice you mean redoing the game or is there is a way to do it through collective mind?

@thecommandofada - I tried using the collective mind with no joy. It seems that using Quick Resume glitches the achievement progress for a playthrough. The only way I could find to get the missing achievements to pop was to start over, which was the same for others posting on Reddit.

This is for healing Lucrecia's Lament, the second-to-last world available to you in the story. The cutesy landscape devolves into the depths of the subconscious where the primal aspect that is now known as Maligula is attempting to breach the locket lock Ford placed years ago. Entering the depths will require you to deal with Censors and other enemies before you must join Ford in removing the lock to deal with Maligula personally, and it is from here we get into the darker depths of the story.
