On the level select, click Menu.
Choose Draw Test.
Keep exiting Draw Test with
and re-entering Draw Test until you see the skeleton figure with the creepy doll head. This is Johnny. Press one final time, and this achievement will then pop.
Please note that Johnny will ONLY appear after you've beaten Special1 Q8.

Great tip, you may wish to add to your solution that Johnny seems to only appear once you have completed Special 1 question 8, (the first time he appears in the game).
I tried hundreds of times before I had completed that question and he didn't show, after I completed the question he popped up in 10 goes.

Same for me. I tried hundreds of times before I beat Special1 Q.8 and nothing. After I beat that level he appeared on the 10th try for me
