No one plays the MP anymore, so start a game with a buddy by going to Multiplayer - Xbox LIVE - Custom Match - Ranked Match, and then use the following settings:
Gametype: Tourney (or Deathmatch)
Game Length: Quickie
Match Type: Ranked
Start the match and then have your friend search for the same thing. You will have to wait in the lobby for 2 minutes before the game starts. It is very unlikely that anyone else will join your game. Then simply kill your buddy 2 times (or 5 if you chose Deathmatch) without them ever killing you and this will unlock.

No. You only need 4 people for the Capture the Flag game type.

Win a ranked game without dying. You and one friend need to search a deathmatch quickie game. The best times are early in the morning when few people are on. You need to kill your friend five times without dying, just have him let you kill him. If people keep joining your deathmatch game, then search Tourney Quickie, it only takes two kills and no one ever searches tourney. Very Simple.
