FOR PC PLAYERS:
From the main menu:
1. select "single player"
2. select "new game"
3. select "Dissolution of Eternity"
4. hit ~ to bring up console command
5. type "map r2m8" (you have to start THIS campaign to be able to warp this level)
6. type "god" and "noclip" and fly to a room with a round thing in the middle. The teleporter is in that but you need to do some stuff to end this level. You will also see a big room with a claw and lighting. I think you need to open that room to activate the exit for the level. So let's do that: back in the room with the round ball thing, there are 4 switches. Simply hit all four of those and the claw with lighting room will open. Kill the claw device and the level will end. Done!

I killed the claw without any switchs and it ended

You'll get this when you finish Dissolution of Eternity. At the last level, there'll be a boss, which is a dragon that flies in circles inside the cave you're in.
To beat this boss, you need to always stay behind it. Don't worry about grabbing all the items and ammo laying around the arena; just stay behind the dragon and follow it closely. Whenever it starts turning left or right, run to the opposite direction so you always stay behind it. You'll have to look up to shoot at the boss, so always take mental notes of where you're walking, otherwise you'll fall in the lava. Also, if the boss turns around and faces you, it'll shoot explosive lava balls (which you can easily dodge) or lightning bolts (which, to my knowledge, are undodgeable and kill you in one hit in Nightmare).
A few loops around the cave should be enough to take the dragon down. Once it's dead, the bars at the cave's end will lift, and you'll be able to go to another room where there's some sort of machine. Interact with the machine to end the level and get the achievement.
