How to unlock the Indigestion achievement in Quake - Definitive Guide
XBOX PC PLAYER only GUIDE:
I REALLY recommend you do this one on nightmare and go ahead and get that out of the way along with this one. There is no point in doing this one separately. If you do the following on Nightmare, both achievements will unlock. Here is the non-nightmare solution anyway:
Hit the ~ key and type "map end" and now you will warp to the end end end boss, Shub-Niggurath's Pit. You will drop into some water. Quickly hit ~ and enter "god". You can just play this part or hit "noclip" and fly to the teleporter at the top of the stairs overlooking the boss creature.
How to kill the boss: The boss is the big barnacle looking thing on the island in the lava. You will notice a flying spike creature. When you jump in the teleporter you will drop out where ever that spike creature is. You need to time it right. When it goes through the boss, Shub-Niggurath, the barnacle, enter the teleporter. You just tele-fragged the end baddy. Achievement yours.
(please note level select seems to nullify some achievements, but using the command code work with no problems)

actually this site is for "achievements" and the website wouldn't be doing its job if it also didn't include the achievement's that were on all Microsoft platforms, including pc,
as stated this game does not have an individual windows 10 stack and shares its stack with the Xbox version. its clear that it belongs here, just because it doesn't help you specifically does not mean it is not helpful to hundreds of other people who use this site.
one of the paragraphs laid out in the "achievement solution guidelines" for posting solutions for achievements states this information, I have highlighted the portion that directly relates as to why this is a valid solution laid out by the site rules and procedures.
"Be relevant. Solutions must be a means to getting the achievement they are posted for. Avoid repeating information that is available from the achievement itself. Give all the information needed to unlock the achievement in question. If you're writing a solution for non-Xbox game (a tablet or Windows Phone), please state which device you've based it upon as not all solutions work for all devices."

i already killed him 3 times and no achievement drop, i dont see what am i doing wrong here.

In order to reach Shub-Niggurath, you must first obtain all four Runes. You’ll get one of them at the end of each Episode. After you have obtained all four, you will be able to enter Shub-Niggurath’s Pit. You will be greeted by a couple of Rotfishs and a Scrag, which you should be able to take out with no issue, even with just the standard shotgun. This section also has a Thunderbolt that I highly recommend picking up, it’ll make the "fight" much easier. Enter the lava portal and you’ll be greeted by a spiked ball. More on it in a bit.
From here, you will be able to claim all of your remaining weapons and start the "fight" with Shub-Niggurath. I say fight in quotation marks since this is more of a matter of reaching a teleporter rather than actually fighting Shub-Niggurath. Instead, you will be fighting a bunch of Shamblers and Vores. Clear them all out and reach the teleporter. You will notice that the spiked ball is floating around the arena, occasionally going through Shub-Niggurath herself. That is the moment you should enter the teleporter. If you time it right, you will tele-frag Shub-Niggurath and finish Quake.
