How to unlock the Machinehead achievement in Quake - Definitive Guide
TLDR (thank you SpeedMasterBen):
1.- New game and pick Dimension of the machine
2- hit ~ and enter "skill 3"
3- hit ~ and enter "map mgend"
4- hit ~ and enter god
5- hit ~ and enter noclip
6- Go to the portal to the east of the lava pit
7- Turn off noclip then go through the portal
I was hesitant about posting a guide for this one so please be warned ahead of time, this one may be buggy. It took me 8 different tries until it popped. I beat the campaign in full and didn't even work. I tried the following methods 3 or 4 times and it finally popped on the last time. Maybe it was just really slow for popping. If you have success or problems with it please let us know in the comments.
XBOX FOR PC PLAYERS ONLY:
1. Start a new single-game on Machine on Nightmare
2. How to start nightmare hit ~ and enter "skill 3" OR the old fashioned way if you are curious: there is a little passage next to the right of the HARD portal. (you CAN make that jump btw, but noclip if you keep falling like me) Also this video at the end show you how. (not my video)
4. hit ~ and enter "map mgend"
5. This is very quick, turn on "god" and "noclip" and the exist portal is to your right about 45 degrees and down about 45 degrees. Hard to miss. Just fly to the portal room. Turn off noclip and jump through.

Figured out a fix for anyone having a issue. I loaded up the Xbox version got to the nightmare mode saved. Loaded on pc and synced Xbox save loaded the Xbox save went to first level via a portal then teleported to Map mgend no clip to end portal and it finally unlocked

3. I am 95% sure you need to now start one other level before level skipping. You have 5 portals to chose from after you jump into the nightmare portal. I don't think it matters where you jump at this point. So choose one then...

I got all the difficulty specific achievements on my first try, huge thanks to GR34TD3STR0Y3R for the awesome guides! Here are the steps I took, hopefully this will help out some gamers that are having trouble getting it to pop.
First off, I never typed the difficulty into the console. I always selected the difficulty from inside the game, such as going through the water in the base game. Then I would always load into the first level normally. From here I would start using the console. I would type in the map first. Once I loaded into the map, I would pause the game real fast because sometimes enemies will be close by. Then I typed "sv_cheats 1" and then "god" into the console. Depending on the map I would either kill a boss or type "noclip" into the console and just fly to the end portal. Before I beat the level, I would always type in "sv_cheats 0" to turn off all cheats.
I also wanted to clarify that this is a play anywhere game. So if you bought it on an Xbox One or Series X|S, you can play it on a PC. It will show up as the same game in your library and will have one unified list of achievements. I can 100% confirm this because I did this myself. You just need to have the Xbox app (and all the system requirements like Windows 10 etc.). You can sign into the Xbox app and simply download the game to your PC, it's only 1 gb. The game will sync perfectly, all your saves will be there and even your settings will be the same.
