This works on any difficulty.
Start up Episode 1, go through the first room, turn left and go down the elevator. Take care of the enemies if you want and then drop into the water at the bridge. Follow the water through a cave on the right to enter a secret area and unlock the achievement.

On Mission 1 of the quake campaign, as soon as you spawn in walk to the next room. Immediately on your right is an enemy and a ledge. Kill the enemy and jump on the ledge. Shoot the wall on the right side and it will retreat giving you a secret area and this achievement.
