How to unlock the Shub's Bud achievement in Quake - Definitive Guide
First, you need to know how to access the Nightmare difficulty. The following is taken from ID's own devs: Nightmare is hidden and can only be obtained if the player knows where to look. To find Nightmare, go through any portal at the start and then walk up the steps leading to Episode 4. Jump into the water and then go back from where the player entered before falling out the bottom. If done right, the player will land on a wooden beam. If he/she misses, then look up and he/she will see the beam and know where to fall on the next try. Follow the beam to the opening in the wall and the player will find a lava portal. Go through this portal to play the Nightmare difficulty. (I believe there is a console command to set the difficulty level, but I think it only works on the DLC not the main quake campaign)
FOR PC ONLY:
Good news for PC players, this is maybe the easiest achievement in the game.
Hit the ~ key and type "map end" and now you will warp to the end end end boss, Shub-Niggurath's Pit. You will drop into some water. Quickly hit ~ and enter "god". You can just play this part or hit "noclip" and fly to the teleporter at the top of the stairs overlooking the boss creature.
How to kill the boss: The boss is the big barnacle-looking thing on the island in the lava. You will notice a flying spike creature. When you jump in the teleporter you will drop out where ever that spike creature is. You need to time it right. When it goes through the boss, Shub-Niggurath, the barnacle, enter the teleporter. You just tele-fragged the end baddy. Achievement yours.
(please note level select seems to nullify some achievements, but using the command code work with no problems)
IF GOD MODE ISNT WORKING: (all credit goes to Chocotoaco119)
According to a recent post on reddit you need to type "sv_cheats 1" hit enter and then type "god". You should get a notif in the console that god mode is on.
MAKE SURE YOU TYPE "sv_cheats 0" AFTER YOU HAVE ACTIVATED GOD MODE OTHERWISE YOU WILL NULL THE ACHIEVEMENT. God mode will still stay active after entering

Does any of this work if you just have an xbox cause I don't got a pc for this

No, I am sorry. Unfourntly they put the PC and Console version achievement list together and no one has uncovered any way to get the cheats to work on the console version. I don't know why NightDive studios did that.

Key information first. You need to start a New Game for this. Doing it from level select won't give the trophy (from what I have read). You don't have to unlock the secret levels if you don't want to. I personally went through all secret levels since I did a full run for all the trophies.
To unlock nightmare difficulty, start your game, pick any of the difficulty and then walk through to episode 4 on the right. You will come to a pool of water, look down drop into it and immediately hold back. This will allow you to float, rather than fall straight through. Look for the wooden beam below and aim for it before going through. After landing on the beam, head through the gap in the wall and through the portal. This will set the game to nightmare difficulty. Video linked for process to unlock it if you prefer to follow the method, rather than read.
To get this trophy, you have select New Game > Quake campaign and play through Episode 1, 2, 3, 4 and the final boss room on the nightmare difficulty. You can save as often as you want so it is just a case of trial and error. What I would do is at the start of each level, I would make a proper save, and a quick save. I would then push through the level and if after each fight I felt I ended with a good amount of health and armour, I would do another quick save. If you quick save in a bad spot, you can either die and let the level reload at the start, or use the proper save. I would save each level as a new save until I ran out of slots, and then start overwriting the oldest. I didn't need it, but the idea was to have past options if I ended up with too low ammo or health to progress.
Use corners for cover when you can. Try to remember ammo and health pick ups when you are full. That way you can backtrack. You carry everything over to the next level, so being prepared is always smart. Nothing carries over between episodes though, so when you are nearing the end, use your most powerful ammo. If there is a quad damage pick up, I would make a quick save before grabbing it. I could then scout out the best route to use it. If I wasted a lot of time going to where there are no enemies, I would quick load and use it smarter next time.

Your melee weapon is fine to use though, so I would say to just take it slow and kill enemies as you go. Use cover etc, and use quick saves. That gives

Below is what ultimately saved me from quitting this game, so hopefully it can do some good in your playthrough. Some general tips, many of these are applicable to the add-ons:
- Mouse and keyboard is an absolute must! Using a mouse is so much more accurate than a controller, and you can quicksave / quickload without having to pause the game.
- Quicksave when it is safe to do so. Quicksaving regularly is also a given with games like this.
- Make a manual save at the beginning of EVERY level, I can't stress this point enough. If you quicksave at the wrong time and keep dying, you can rely on your beginning save and start over.
- Certain weapons seem to work better with different enemies. I would often use the nailgun / thunderbolt on Shamblers and the grenade / rocket launcher on Vores. Zombies need to also be gibbed with grenade launcher / rockets. Double barrel shotgun is handy for Ogres, Fiends and standard Knights, although Death Knights would be better dealt with using the nailgun.
- Use your surroundings to your advantage and always keep on the move during a fight, especially against Fiends pouncing at you, Vores which throw explosive orbs at you and Ogres shooting grenades.
- I didn't personally rely on finding secrets, as many of them lead to traps which can quickly overwhelm you if you're not careful.
- CHTHON: Appears in E1M7: The House of Chthon (he also appears at the end of the "Dimension of the Machine" mission pack. He is a large red demon-like monster who rises from a pool of lava and throws rocks at you which explode on impact and will almost always instakill you on Nightmare difficulty. You defeat him by using the two electric coils hanging from the ceiling, activating them when Chthon is inbetween them both will cause him to take damage. You will need to reposition the coils and damage him a total of three times before he is defeated.
- SHUB-NIGGURATH: Appears in Shub-Niggurath's Pit (this is at the end of the game after you collect the 4 runes). She is a cosmic entity and is also known as "Quake", her body mass oozing with blood and she has three large tendril limbs connected to the body - sort of remiscent of the game logo. While Shub doesn't do any damage to you directly, she will instead spawn waves of Vores and Shamblers to fight you in the pit. You simply follow the path along once it is safe, and come across a portal which leads into the large pink orb floating around the pit. Wait until the orb is coming towards Shub and quickly jump into the portal to instakill the monster, beating the game and awarding you at least 86G tied to this achievement.
