On E1M5: GLOOM KEEP (Easy), going into the large building in front of you when you start you'll eventually come to a choice of going left or right.
Go right and clear out the area ahead of enemies until you reach a portal. You can enter the portal from the side you first see to go to where the Shambler is, or the other side of the portal to go to an area on top of the building you entered.
On top of the building you'll find Quad Damage. I would recommend playing through other parts of the level to stock up on ammo for the Super Nailgun which can be found downstairs from the portal, nearby where the Ogre lurked.
Grab the Quad Damage and (as a commenter (Juju) mentions) head back through the portal on top of the building and you'll be taken to the Shambler. The Super Nailgun should make quick work of the Shambler before he's able to get a shot off.

(Normal) I got this one really easy on the secret level Ziggurat Vertigo (One of the secrets on the level Grizzly Grotto)
At the end of the level just before you find the silver key, there's a Shambler on the roof. And on a platform below in the right corner, there's a Pentagram of Protection. Before you pick that up save the game, and after that take him out with no bigger effort. I used the Super Nail gun. Good luck!

for PC follow this solution but add this command: "give"
I did it with "give 5" then "give n 500" for the Super Nailgun with 500 ammo.

hi this is my first achievement guide video i hope it helps
Here ill be putting steps for the achievements as well:
1: Load level E2M2 the Ogre Citadel normal/medium difficulty (i recommend just playing E2M1 first to get some ammo)
2: After you clear the room with the 4 knights in the make a right and go down the stairs and follow the path to the nail gun and quad damage save before picking up both
3: jump the gap grab any health kits you need and drop down into the room you can get both achievements in one run by meleeing and killing the shambler before it casts lightning but if you dont get it doing it that way then just reload the save and use the nail gun with quad damage to make very quick work of it

The best level to get this is in Quake E1M5 Gloom Keep. Try to save at least 30-50 nails and have the super nail gun. Continue through the level until you reach a portal. Enter the backside of the portal to be transported to a secret area. Kill the enemies there, grab the quad damage, bring out the super nail gun, and return through the portal. Grab the key and you'll be pushed up and have a shambler in front of you. Immediately start firing on it and it should die in a couple seconds, netting you the trophy.
Did this on Nightmare so it should work regardless of difficulty.
