XBOX FOR PC PLAYERS:
From the main menu:
1. select "single player"
2. select "new game"
3. select "dimension of the machine"
4. hit ~ to bring up console command
5. type "map mgend" (you have to start THIS campaign to be able to warp this level)
6. type "god" and "noclip" and fly to the exist portal. It will be down and to the right of the boss. Exit noclip mode and jump through the portal.
(NOTE for PC or Console players, the level select from the main menu will nullify achievements)

Just a heads up, but the command "skill 3" will also pop the nightmare difficulty related achievement upon completion of the level (was a bit delayed for me however). I assume this should work with all campaigns too and eliminate the need for finding the nightmare portals but I cannot confirm. Noticed you had a solution up for all other nightmare achievements but not this one yet. Hope it helps!

Thank you. Yeah. I knew there was a command for it but wasn't sure what it was. I read that it won't work on the QUAKE campaign, but will work on the extra campaigns.
