For all upgrades you have to collect 60 Chronon Sources. Chronon Sources are part of the over 200 collectibles in the game. The first Chronon Source can you collect in Act 2-1 and the last in Act 5-3.
I will link here the Chronon Sources from my full collectibles vid for each Act. If you prefeer a foren thread then take a look here.

Once I had gathered all Chronon Sources, on Upgrades it said that, 1 more Chronon Source(s) for next Upgrade Point, was required, even though this was not true. I had simply to initiate the process for purchasing the final Upgrade.

If I have missed one (which I have) will I have to play the whole campaign again? Missed one in act 2 as I died and forgot to "recollect" it. It should auto save every time you pick one up. Annoying.

A COMPLETE guide of all 200+ collectibles with timelinks can be found HERE.
The collectibles, including 60 chronon sources required for upgrading time powers, are found within Act 2 through to Act 5. They are found using the time vision ability near them to reveal and collect them.
Upon picking up the last chronon source, you will can buy the last upgrade and be rewarded with the achievement.
You can replay chapters using the timeline at any point, but keep in mind that changing your choices within the Junctions will add new collectibles for you to collect, bringing your percentage down. You can quit to the main menu once you collect them and reach a checkpoint save.
Lastly, use the menu to check "Collectibles" & "Upgrades" where it'll show your collectibles for each Act/Part so you can easily keep progress.

The Media Baron achievement in Quantum Break worth 52 pointsYou need to find all Emails, presentations, TV Shows, Radios And Posters

I would suggest starting off with the solution for the upgrades first, and then finishing with the link to the full collectibles. It is kind of irritating when people post guides for several achievements under one and then copy and paste it, which is what it seemed like you did until I read further, espcially because the video is just that.

Nach so vielen englischen Guides möchte ich natürlich auch mit den deutschen nicht nachstehen.
Zum Upgraden benötigt Ihr alle Chrononquellen die über alle Akte verstreut sind, das Spiel hat insgesamt über 200 Sammelobjekte wo und wie Ihr die findet ?
Mit meinen Videos, in der Videobeschreibung sind auch die Zeitangaben zu jedem Sammelobjekt enthalten so dass Ihr direkt zum fehlenden Teil springen könnt

I didn't vote neg because of the language, I did it because you should be addressing the solution to this acheivement, and not simply spamming a single collectible guide for all achievements involved. sorry. not a good guide for this achievement.

I wrote down the Solution in German that you need all Chronon sources to get all your Timepowers upgraded and then i posted the Vids which include the Timeline for every Chronon source.

To fully upgrade all of your time powers you will need to get all 60 Chronon Sources in the game. Collecting Chronon Sources will grant you upgrade points. To get into the upgrades menu, press 
then 
to access the Diaries & Upgrades menu. You will have all of them by Act 5 Part 3 before you and your brother enter the pool.
- Time Vision: Active Pulse - 1, Increased Threat Detection - 4, Increased Range - 6
- Time Stop: Increased Strength - 1, Increased Size - 4, Extra Activation - 6
- Time Dodge: Increased Focus - 1, Maximum Focus - 4, Extra Activation - 6
- Time Shield: Recovery - 1, Slow Time - 4, Increased Power - 6
- Time Blast: Increase Blast Radius - 2, Reduce Cooldown - 6
- Time Rush: Increased Focus - 2, Multi-Takedown - 6
