At the end of Stage 3, you are asked to choose Hatch or Amaral.
Selecting Hatch will award The Right Hand Man achievement, selecting Amaral will award The Doctor.

I seem to have the same issue as Sir Funkified, achievement won't pop. Even started a new game and played up to this point with no difference after I completed the game choosing the other junctions.
Edit #1: Checked today and the achievement popped for me. Said I unlocked it 2 days ago. There must be some serious lag with the Xbox achievement server for some of these achievements.

Seems to have glitched for me.
I've done this Junction twice now and chose Amaral but the achievement never popped.
Did it another time and chose Broyles and that one popped ok.

This achievement is earned at the junction at the end of the third act where you play Paul again. During this junction you need to decide whether you want to trust Martin Hatch of Sofia Amaral. This achievement is earned for choosing to trust Sofia.
Note that you can make this choice, then immediately replay this section and make the other to unlock both achievements if you don't want to worry about replaying it toward the end of the game. Make sure you do not replay a junction choice until you have completed all of the collectible achievements since doing so will change/move many of the collectibles, essentially requiring you to refind them.

This is obtained during Junction 3. At the end of the junction you will have a choice to either trust Martin Hatch or Sofia Amaral. Press 
to choose to trust Sofia Amaral to obtain the achievement. After the achievement unlocks, press 
and select restart checkpoint, then choose the other option to unlock “The Right-Hand Man”.
