I beat this whole game but for some reason this achievement never unlocked for me. Sucks because it's the only one I need for the 100%

Also completed the whole game and this one didn't unlock. Such a bummer.

This one didn't pop for me either, but fortunately it unlocked just by replaying the last chapter of the 4th act.

Solution to any chapter-completion based achievements: Navigate to manage game and add-ons, go to saved data and delete all, launch up the game again and allow your data to sync, then replay the last chapter of the act you need the achievement on.
