Right above the cellar door there is a guy on the balcony headshot him and kill him in one shot and the achievement will unlock.

when you near the balcony, you will see a wall that is perfect cover for this acheivement. just pull out a pistol that you can spare some ammo, like the P99, and just if your aim is right, and its most likely it is, you would have shot the guy in the face and, bleep bloop, the acheivement is yours!
edit: its useful if your playing on the 007 difficulty because its the safest option, just in case if your doing that

On the first level, White's Estate, there will be a guy on a balcony shooting at you. All you have to do is shoot him in the head, and that should kill him in one shot, resulting in him toppling over the edge through the cellar door.
Savenger
As you approach the mansion in the level White's Estate, a lookout will shoot at you from a balcony on the right side of the building. Take him down with a single shot, and he will fall straight down. Do this right and he will open the cellar door for you, using his head as the key.
Remember: The single shot part is the key to getting this trophy, so aim carefully.

This is on the first level of the game. After you pass the big green house, you will have to go around the mansion to the right. There is an enemy on a balcony and you must get a head shot on him. Once you get a head shot, he will fall off the balcony and break through the door below him.
