You can get it by yourself (I personally did it) just plugging a second controller, and playing normally. Once you see a powerup or the second player spawn, make him crash, and pick after that the powerup. He will respawn. Go kill him again, and repeat.
If you are playing with a friend, live all the powerups to one of you, and make the other one die. If both of you die, you will respawn on a checkpoint, and sometimes isn´t that good to do so.
If you play without dying until Stage 3 Boss, you can suicide both players, because you will start at the beginning of the level, where there are 2-3 easy powerups in less than a minute. You could try Stage 4 too, but it´s easier to get killed before getting it.
Just don´t forget to make one player die before picking any powerup, and surviving player should be able to survive by himself.

"Just to add to Metalminds solution, i found it best to have the other controller on the floor with my big toe on the analog stick pushing either down or up since theres walls on the top or the bottom of all the stages the other player will die instantly after the spawn protection wears off and you dont have to fiddle around trying to make them crash."
Once you collect a power up (doesnt matter which one, speed, weapon, missle, etc) it will revive the second player and just rinse and repeat over and over and you should have it by the 2nd stage.
My partner FrozenLionheart and I just did this early this morning, and we discovered that you can't do it after losing all your lives and expending a continue.
Getting to the strategy, one player will get all the power-ups but absolutely MUST survive until the level three Boss music begins. The other player will be the sacrifice, dying each time a power-up appears. Once the power-up appears, it will be solid, but once a player dies, it will begin to flicker, meaning it will be ready to revive the lost player. It is absolutely vital that you wait until the power-up starts flickering to pick it up and revive the other player.
Now for the levels. In the first part of level one, you'll get one three-way laser power-up. In the second area, and to the end of the level, you'll find one ring laser power-up, one missile power-up, one or two more three-way laser power-ups, one overhead shield-satellite, and one speed-up power-up. By the end of first level, the powered-up player will have the level three three-way laser, homing missiles, the level three front shield-satellite, the overhead shield-satellite, and one speed power-up. You can quickly kill the first boss by staying low to the back of the room, and raising up after the Boss' weak point spits out the strand of insta-kill rings and his is down, and firing powered-up blasts at the weak point. After level one, the powered-up player should have revived the sacrifice seven times.
On to level two. Keep the powered-up player about three-quarters of the way out front and the sacrifice towards the rear of the screen, as the powered-up player will be luring out the scorpions for quick-kills with the level three three-way laser and missiles, but be cautious in doing so. And when the green pods come out of the floor and ceiling, have the powered-up player nose up to them with the front shield-satellite and just wait them out until they explode. During this first area of level, watch for a speed-up and a groundpounder power-up (don't worry, you'll get the three-way laser back in time for the Boss and level three). After the checkpoint, watch for the first Boss snake and two three-way laser power-ups, and take out his weapon balls quickly to avoid the powered-up player getting skragged by accident. After the first Boss snake is disarmed, stay out of its way, but get the powered-up player in position at the full Boss location to situate in between the top-left emergence port and the Boss' top-central eyelid, positioning the front shield-satellite directly on the eyelid so that the three-way laser can still shoot the front and top beams at the second-spawning Boss snake overhead. The satellite-shields will keep that player protected from shots and the snake itself. Total revives in this level should be about four, totalling eleven if you got 'em all in the first level.
Going into the third level, hopefully the powered-up player is still fully juiced, as the third level is going to be literal bullet hell compared to the swarm hell in level two. Stay close to the center, but not exactly center, as the engine of the Bydo battlecruiser will fire blasts directly center behind it. Take that out quickly, and two groundhugger power-ups will spawn before you're finished with the rear of the cruiser. After you've removed all the top and bottom weaponry from the rear of the battlecruiser, you'll start battling under the ship, where a ring laser power-up will spawn, and this will quite possibly be the last power-up you can obtain unless you can somehow get one of the two either at the top of the ship or at the front of the ship once you've made to that end of the ship before dying and restarting the level. The top one's impossible, but there's a slim chance to get the front one if you take out the front guns on the battlecruiser and watch for the power-up bot to spawn. As long as you get three power-ups per life for this level each life (you should have an extra life totalling three by now), you should be able to max out at nine total revivals in the third level, totalling twenty over the three levels and dying three times during the battlecruiser fight. Once you get your twentieth revival, the achievement will pop for both players immediately.
This WILL take a LOT of practice with your partner, and you WILL need to do it in 1CC, or one credit completion for non-arcade players to understand, continuing will NOT work!!! This can be done in under an hour including practicing and memorizing enemy spawns and patterns, and as long as the powered-up player STAYS powered up until the end of level three, and you repeat the three power-ups in level three a couple times, you'll have the achievement.
You'll need a 2nd controller for this. Start a multiplayer game in Classic mode. In Classic if a player dies they will be revived the next time you pick up a power up. Let one player die then play as normal collecting power ups as you go, which will revive the other player. Repeat 10 times to unlock the trophy.
In Classic Co-Op mode both players share lives. When one player dies, the other is given the opportunity to revive the other player by either finishing the stage or by collecting a powerup. A life is not lost until both players ships are destroyed.
Try to remember where the powerups come along, and have your friend run into a wall or an enemy when you are about to collect a powerup in order to get 20 of these in a single co-op game.
