How to unlock the Heavy Boots achievement in RAGE 2 - Definitive Guide
This is not an NG+ mode, so you are better off doing this on Easy difficulty, unless you are going for one of the other difficulty achievements from this Title Update (absolutely not recommended!) You will pick up the Assault Rifle and Shotgun along the way as part of the main story - between those and the grenades you won't need anything else if you're playing on Easy.
You can pick up the defibrillator in the 'Canyon Cove' Ark in the south eastern part of Broken Tract, which is likely the second ark you will come across. I never needed to use the defibrillator but used Health Infusions many times. You may want to pick up 1-2 Health Infusion Upgrades (1 is found in each Trade Town, and the trader you will encounter on the roads also has one) and pick up the upgrades in Kvasir's upgrade tree which let you hold 6 (or 8 if you upgrade twice) infusions in your inventory.
You need to get Marshall, Loosum and Kvasir to Level 5 in their respective projects to finish the game. Kvasir will likely happen naturally as you move through the map and come across data pads and any other icons with blue labels (such as fallen rangers.) Loosum requires you to complete orange locations, such as Automated Sentries, Gas Stations and Road Chokers. Marshall requires you to complete pink locations, which are mostly bandit dens. Level 5 for each person is 500 points, so you can pick harder targets for Marshall & Loosum to get to 500 with fewer locations that need to be explored.
In the final mission you will be driving a tank through what is basically an auto-scroller, even on Easy you will be shot a lot, and dying in the tank gives you no defibrillator option. If your tank gets to half health, quit out and load the Iron Man Mode back up, where you'll be back in the tank at the previous checkpoint at full health, so you can try again. Targets in this long series of corridors (tanks, enemies, flammable barrels) spawn from fairly far away so you can shoot enemies from a distance to stay safe.
Be careful when in free roam. At one point, driving (or falling by foot) off a cliff is an instant death. As of 2021 it appears that if you do fall off a cliff, you are taken back to a previous checkpoint and you don't die or lose your save.
A fast run through this game mode without unnecessary exploration can be done in 5-6 hours. The achievement will pop after the credits are a brief cut scene with the major characters in the game.
(H/T to Scotty x360a for some details provided in this solution, and Mack Stardust for the updated status of the risks driving/falling off cliffs.)
If you don't see this, go to your saved games and look for an autosave that says "Ironman" next to it. If you don't see the flaming heart icon and you don't have an "Ironman" autosave you should start over because you're no longer in Ironman mode.
While playing, you should see a flaming heart icon in the bottom-left corner of the screen denoting that you're in Ironman mode as in the screenshot below.
Also good to note that you do not have to do the Sandworm or Mech Retrieval missions during your Ironman playthrough.
To unlock this achievement you need to complete the game in Iron Man Mode. You have to choose this game mode from the main menu and it is a separate option from normal new game modes. You cannot save the game in this mode and if you die, your save will automatically be deleted and you will need to start the game again from the very beginning.
However, fret not as this can be done on the Easy difficulty. You can complete this game, simply going for this achievement, in around 10 hours, even less, however I would recommend spending some time at least getting some useful skills and weapons to help you along. It is very highly recommended to get the 'Defibrillation' skill that can be found at the Canyon Cove Ark in Broken Tract:
Acquiring this will give you a second chance should you accidentally get yourself killed. However this only relates to combat; if you die in a vehicle, either from it being destroyed or by driving over a ledge into an unplayable area, you will die immediately and ruin your run.
As with Ultra Nightmare, make use of the Health Infusion and don't be afraid to use them as you can always get more of them and the ingredients to make them from the same vendor in Gunbarrel. Also put your Feltrite into upgrading your Constitution from the upgrades menu as this can reduce damage from bullets and explosions. You can also have your health upgraded in Wellspring but the ingredients you need to do so are somewhat difficult to come by and you only get 5 more health per upgrade, so it's not entirely worth the effort.
It is worth noting that you cannot save the game, however you can exit to the main menu which will put you at the nearest discovered location or last checkpoint when you load the game back up, so you don't have to do this in a single sitting. This too is a viable option, when you load it back up at a checkpoint you will have full health, so for example on the final mission when driving the Xerxes tank that has a tendency to be easily destroyed and the potential to ruin your run, when your tanks health starts to get low, exit the game, then load it back up and you will be at the latest checkpoint and all your health will be back, giving you a better chance to progress. Don't be afraid to abuse this should you see fit.
Also it is finally worth noting that you cannot start a concurrent playthrough of this game while attempting this game mode. Since you cannot save it, the only way to return to it after quitting out is via 'Continue' on the main menu. If you load up another save file, then save the game or trigger an auto-save, your current Iron Man Mode will be overwritten and you would need to start it again, so when you choose to go for this achievement, don't play any other aspect of the game until it is done.

This can be done on Easy but if you die, all your saves will be erased so make frequent backups! Luckily the campaign is very short and can be done in around 5 hours so if you do mess up, it's not the worst thing in the world.
Tips:
- The game autosaves which can be reloaded so no need to worry about crashes or playing the game non stop.
- Falling into the trenches does not kill you, it just respawns you nearby.
- Ironman must be played on an entirely new save file
- Be careful driving non phoenix vehicles if they explode you're back to square one. Phoenix does not explode when damaged.
- There are a few glitches still so be careful and upload your save to the PS Save File Cloud or a USB Drive on a frequent basis.
But there are two tricks:
The difficulty level can be reduced to minimum.
You can backup to a flash drive or PS+ cloud.
As a result, there should be no problems with this trophy.
Before turning in the last quest for Marshal/Luzam/Kvasir, after which you will attack the government, make a backup copy of your save! Precisely before submitting the quest, otherwise if you make a backup copy after submitting the quest, but before starting an attack on the government, you may experience a bug when the task does not appear and you will have to redo it all over again (as happened to me).
