In Arcade mode your settings should be as follows.
Game mode: Normal
Difficulty: Practice
Medals: your preference
Credits: doesn't matter
With this set up play the game normaly and don't die. Nothing shoots at you so it is easy to just fly and shoot. The Achievement will pop up in one playthrough.

Complete Normal Mode without a continue. Set the difficulty to practice. With the difficulty set to practice then you just have to make sure you don't run into anyone as no one will fire at you.
You can do this just by holding down rapid fire and sitting in the middle of the screen most of the time. You can lose a life (1 of your 3), but you can not use a continue.
