How to unlock the It's Pointy! achievement in Ravenlok - Definitive Guide

MISSABLE!!
After defeating Thorlaron the Hydra you will be teleported to a blue forest area, if you head straight ahead you will see a thick "tree trunk" with glowing blue eyes, to the right of this tree is a blue chest, open the blue chest to receive the Witch's Hat. Hit cn_start to open you menu, tab over cn_RB to "Hats" and equip the Witch's hat. Achievement will pop, hats do not have any special abilities so wear whichever one you want.
This is a one-time area, if you did not open the chest after defeating the boss you may need to replay a new game. So if you've finished and are "mopping up" you will need to start a fresh game and fasttrack to this point.

Is this glitchy for anyone else? I’ve been wearing it for about 20 minutes and nothing has popped.

Is there any way back to the hat if you missed it? I walked right past it into the trunk to get the relic, now can't find a way back to the chest. Maybe this one is missable?

The area with the Hat is definitely missable. The only way to get to it is killing the three headed Hydra in the teacup and asking the snake(?) like monster to transport you there. However he only does it once / until you pick up the mandatory relic.

The witch hat is found in a chest directly after defeating the hydra boss. This and all of the following accessories aren't required as part of the story to wear like the masquerade mask, so be sure to equip it to unlock the achievement.
