This can be unlocked on Jester's Trick House. (Book 6, Red/Blue)
The area is located close to the middle of the level. You will be drop down to an area that looks like the image linked below.

Kill the Demon on the left with your ice powerup, but watch out for the projectiles that you have to jump over on the right. You want to get all the way across to the left without falling in the water, if you fail, you can die and restart from checkpoint.
The first 3 blocks activate if you jump on them and the last one is on a rotation, so time the rotation so that the hand isn't out when you get to it. Then jump to the pink block, but stand closer to the right, as there is another demon that you kill with your ice powerup. Then the last group of "hand blocks" are on a rotation, wait for the first one to pop up, then follow it across. Achievement pops when you jump off the last block.
