This achievement is also a bit RNG and you need to go out of your way to get it. This might take some tries, therefore I strongly suggest quick play instead of career/Commander.
Just a handful of civilians in the whole game are agro, despite of you not doing anything to hurt them, but they do feel the need to kick your ass. That being said, you must not shout at them to get down, neutralise or kill them or do anything which will make them afraid, just walk up to them.
One such NPC for this is in Valley of the Dolls. From the entry point, tell your team to hold position (they tend to cuff anybody who has surrendered) go straight and through the door. There might be security there, so deal them as you wish. In this room, go through the back left door. You should be in a long room, with a bed that's... been slept in at the other end. Near the bed you'll find Voll's wife, Daniela, a blonde Romanian lady in a white dress, who's super pissed you came in to serve that arrest warrant on her husband. She's not always in that part of the room. She might be near the security personnel you stumble upon after going through the first door or on the balcony of the long room.
Provided she hasn't been spooked, walk up to her and simply let her punch you to death. Keep walking into her, it takes 5-6 hits for you to die. If she's kneeling, that means you've intimidated her, so you'll need to restart.

Play through the campaign until you've unlocked Valley of the Dolls. Upon starting the mission, pick the lock of the front door, but send your squad down the stairs to the right or hold position. Go through the first door on your left and you'll find the woman with the long blonde hair. She'll either be hiding in the closet or be in the next door along the balcony.
Don't shout at her or fire your weapon, just butt up against her enough times for her to get annoyed and hit you. She'll hit you enough times to kill you outright.
If she goes into the submissive stance that you'll be able to cuff her, she won't attack you, so you'll need a restart.
