This achievement is weird and seems to vary for everyone, so this is what I did that unlocked it.
My Kit-
357 and ammo
primary with no ammo
no grenades, tools, etc
battering ram (not used)
no helmet
no armor
AI Team-
whatever you want is fine for them, but do not let them use grenades or tools, so to be safe remove them from their kits
Strategy
I just let the AI move and clear each area, if there was a door, simple open and clear, if it was locked, kick and clear. Make sure you shoot at least one enemy with the 357 to be safe, but let the team do the heavy lifting!

When building your load out, nothing matters that you select except for your secondary.
Make sure you select the .357 magnum and only use the magnum.

this one is definitely buggy. tried a lot of time doing nothing but just shooting with magnum and no achievement

I finally figured this one out. I don’t know if it’s a bug, bad design, etc., but you absolutely CANNOT change the pistol out of your hands for anything once you equip it. This includes opening the tablet, lock picking, handcuffing, and so on. My teammates were all using a lethal load out, although none of them had shields or launchers as I had read that may affect the achievement.
My loadout was heavy armor, helmet, ballistic mask, anti-flash goggles, and all my munition points into pistol ammo with no deployables. I didn’t wear NVG as I also read that could affect the achievement. I had zero attachments on the .357. My long tactical was mirror but I never pulled it out.
The ‘mistake’ I kept making was checking my objectives via the tablet and I believe that was voiding the achievement. Just keep going until the mission auto completes. If you think you missed evidence or something, just use right bumper to send your team to search and secure and they will find it at the end. You’ll know when there are no more hostiles when you are notified you “bring order to chaos.” Then you can freely look for missing evidence or civilians.
I should add that I picked up both soft objectives, opened unlocked doors, utilized low ready to move faster, and shouted commands and this did not void the achievement.
