This is easy by using the storm. In earlier stages the storm does not deal a lot of damage. Let it down you and then wait out the 20 seconds for the revive.

You'll net this achievement if you get damaged enough to be turned into a chicken, then survive long enough to return to the human form.
Best way to avoid being finished off is to run around as many corners as possible, jump through windows and generally go full evasive.
The timer is a little over 15 seconds as default, but there are runes available that can alter it, but you shouldn't need it and will find this will come naturally as you play more of the game.
