When you first get access to using alchemy in the blacksmith's shop try to create a new weapon or something. The blacksmith may end up creating something totally unrelated to what you initially intended. So basically continue to create as much as you can until he creates something completely unrelated.

you will also unlock "Alchemical Aptitude" if you get this on your first creation.

the Vessel of Life sells for 50,000 gold, so Lag is right, you can buy materials and continue to make items until you get "Oops". I suggest buying 4 Vessels and selling them, going back to the Adventurers guild and getting the first couple money related titles. up till the 200,000 gold one. ^.^

This is a random occurrence that can happen at any time when you’re performing Alchemy at the Blacksmith Guild. As you perform the alchemy, it will be interrupted with the blacksmith's face and it will produce a completely different item from what was intended. There are no factors that influence this happening, but because you'll be performing alchemy quite often in the game, this should come to you naturally.

When crafting, there is a small chance (around 5%) that you will fail to make an item and instead create an entirely random item. You will likely get the achievement naturally through normal gameplay, but if you have yet to unlock it, simply buy 99 Long Sword and 99 Silver Bullion (both early craftable items registered to the shop) and make Silver Swords until it unlocks, then reload your save.
